At Cholla Medical Group-Primary Care, our goal is to provide you with the best patient care and service. We have created this section to address some of the most common questions that our patients call us with.
Patient Forms
Before coming into our practice, please download the Cholla Medical Group New Patient Packet and bring it with you to your first appointment.
How do I get my medications refilled?
Upon physician approval, your refill requests will be processed in 3 business days – please factor that into the timing of your refill requests. Narcotics, anti- anxiety medications, muscle relaxers, anti-insomnia, and similar medications WILL NOT be called into pharmacies after hours, on weekends, or on holidays by the on-call provider. Monthly narcotic refills require monthly visits and/or referral to a pain management clinic.
Can I get blood work done in your office?
Yes, blood work can conveniently be done in our office. Please call for an appointment. Hours are Monday through Friday from 8 a.m. to 3 p.m. (closed for lunch from noon to 1 p.m.). Please check with your doctor to see whether you need to fast prior to getting your labs drawn.
Where can I get x-rays or other imaging tests done?
AZ-Tech Radiology is located nearby but they also have other locations that may be closer for you. Please visit their website for the most convenient location.
AZ-Tech Radiology (AZ-TR) – www.aztechradiology.com
How do I get results for labs, x-rays, or other tests my provider has ordered?
Results of your labs and tests will be called to you, whether they are normal or abnormal. Information will be communicated to you directly.
How long does it take to get results of labs and other tests?
You should be notified by our office of results within 1-2 weeks of getting routine labs and tests done (less routine tests may take longer). If you have not been contacted by the office within 2 weeks of getting routine labs/tests done, call the main number to inquire about your results.
Can a family member call to receive information for me?
Information will not be communicated to relatives or others unless you have specifically requested this in writing (this ncludes spouses and parents of legal-aged children). This is to maintain confidentiality. The phone number you leave as your contact number indicates you are willing to receive confidential messages regarding your healthcare at that number unless you specify otherwise in writing.
Do I need to schedule an appointment or do you see patients as a walk-in clinic?
Please call (480) 418-7246 to schedule an appointment. We do our best to get patients in as soon as possible, but since we follow a timely schedule, we do not accept walk-ins. Many times, same-day appointments can be accommodated. If it is a medical emergency, you should go to the emergency room or call 911.
How much will my visit cost?
Cost of the appointment depends on your insurance. A quick phone call to your insurance company can help provide this information. Co-pays are due at the time of the visit. If you are a self-pay patient (if you plan to pay without the use of insurance) we will work with you to determine an appropriate payment schedule.
